Purchase of Insurance

The health insurance for the following types of students will be purchased by the students themselves:including self-funded students, students with expired scholarship, advanced students,language students, and exchange students.

Students should complete the insurance purchase and payment prior to registration and visa application. Students' visa extension period shall not be longer than the period of insurance validity.

How to purchase:

 Insurance Claims Process

1、List of materials required for outpatient claim

A. Copy of the passport

B. Original invoices(Your name on the invoices should be the same as your passport)

C. Medical record(same date with the invoices), and copy of examination report and laboratory test report of each respective treatment, etc.

D. Reimbursement Application Form

E. Certificate of the accident with signature/CIS' s stamp(when/where /how, etc.)in case of an accident, (if it' s a traffic accident, the traffic unit should issue a liability confirmation of traffic accident)

F. Bank account information form (You are suggested to go to the bank with your ID and debit card, and ask the staff there to print the account information form for you. The information should include the account name, account number, and sub-branch name.)

A. Copy of the passport

B. Original invoices(Your name on the invoices should be the same as your passport)

C. Reimbursement Application Form

D. Copy of hospital discharge summary or medical record of hospitalization

E. Certificate of the accident with signature/CIS' s stamp(when/where/how, etc.)in case of an accident, (if it' s a traffic accident, the traffic unit should issue a liability confirmation of traffic accident)

F. Bank account information form (You are suggested to go to the bank with your ID and debit card, and ask the staff there to print the account information form for you. The information should include the account name, account number, and sub-branch name.)

3、 Process to apply for Advanced payment service for inpatient treatment

A. Call 4008105119 to apply for hospitalization advance payment service.

B.  Follow the customer service' s instructions to send the E-mail of authorization and a photocopy of your passport to RYCY@LHGJ.NET
